Clinicians should consider prior fractures, age, risk factors, and treatment response when adjusting therapy, with anabolic options (e.g., teriparatide or romosozumab) prioritized for very high‑risk patients and frequent densitometry used judiciously given measurement variability and limited added value for all patients.
