Tennessee report flags pharmacy failures after four patients mismedicated during surgery at Ascension Midtown
An 80-page Tennessee Health Facilities Commission report faulted Ascension Saint Thomas Midtown’s pharmacy for errors that caused four patients to receive the wrong medication during surgery, resulting in adverse reactions (including a paralytic case) among seven surgeries. The Tennessee Bureau of Investigation is probing the incident; the hospital self-reported the Aug. 14 error and has since implemented safety and process changes, while state regulators continue to review.
