The Crucial Role of Pharmacists in Care Transitions

Transitions of care (TOC) can increase the risk of adverse events related to communication about issues such as discharge medications. Pharmacists can play a key role in TOC to improve patient health outcomes by providing follow-up care, medication therapy management, and patient education. Studies have shown that pharmacist interventions during TOC can significantly reduce readmission rates and save money for facilities. The Joint Commission recommends involving pharmacists in the medication reconciliation process, as they play a major role on interdisciplinary teams conducting clinical interventions.
