SpaceX's Starship rocket exploded during its first full test flight due to a failure of several of the rocket's engines to fire up as planned, followed by a minute-long tumble after the rocket's upper stage and booster failed to separate. The rocket's breakup was triggered by the flight termination system (FTS), which is essentially explosives built into key areas of rockets and is automatically triggered if a hardware failure is detected. The FTS is a safety and licensing requirement and is especially important if a rocket appears to veer off course.
