The US National Transportation Safety Board's initial findings into the incident on an Alaska Airlines Boeing 737 Max in January reveal that four key bolts meant to secure an unused door to the fuselage were missing, causing the door panel to move out of position and break away from the aircraft shortly after take-off, leading to a rapid loss of cabin pressure. The door plug, manufactured by Boeing's supplier Spirit AeroSystems, had been removed and reinstalled in the factory with at least three of the four locking bolts not put back in place, suggesting the bolts were missing before the incident.
