Alder Hey Trust Admits 24 Failures Led to Toddler Eleanor's Death
Alder Hey Children’s Hospital has accepted full responsibility for 24 care failures that resulted in the death of 21-month-old Eleanor Hazel Aldred-Owen. The toddler died in October 2023 after a routine skull surgery, with an abnormal chest X-ray missed by staff. The trust admitted the death was preventable and has introduced new safety protocols to prevent similar tragedies.
