"Ukraine accuses Russia of blowing up dam, counteroffensive launched"

Ukraine's Kakhovka dam and hydroelectric power station has caused massive flooding and destruction in the southern Kherson region, with Kyiv and Moscow blaming each other for the attack. Meanwhile, fighting is escalating in Ukraine's east as the country's long-awaited counteroffensive gets underway.
