Deaf patient receives wrong jab after sign-language gap, ombudsman finds

A deaf patient was given a Covid vaccine instead of a flu jab because no British Sign Language interpreter was provided; staff misinterpreted the patient’s request and obtained consent via her grandmother, despite the patient’s early-stage dementia. The Parliamentary and Health Service Ombudsman urged apologies and awarded £450 in compensation, noting accessibility gaps in NHS and social care communications and calling for better reasonable adjustments. The report also highlighted improvements at some trusts, and stressed the need for accessible formats and interpreters across services.

"Shocking Investigation Reveals Over 400,000 Victims of Church Abuse in Spain"

Spain's ombudsman has released a damning report following an 18-month independent investigation into sex abuse by clergy members and others connected to the Catholic Church in the country. The report suggests that the number of victims could be in the hundreds of thousands, based on a survey that found 1.13% of adults questioned said they were abused as children by priests or lay members of the church. The report calls for recognition of victims, the creation of a state fund for compensation, and reforms to prevent abuse and compensate victims. The investigation was prompted by abuse allegations involving over 1,200 victims published by Spanish newspaper El Pais.
