Players of Diablo 4 have reported a bug with the Holy Chalice item, which is linked to a questline involving Sister Octavia in Kyovashad. The item either constantly appears at the gate of Kyovashad or constantly pops out of the player's inventory, which can prevent them from completing certain quests. The cause of the bug is unknown, but some players have suggested clearing other quests around Kyovashad or teleporting back to the area as potential fixes.
