New footage released by the NTSB shows the UPS MD-11F cargo plane erupting in flames after an engine detaches on takeoff in Louisville, killing 15 (3 crew and 12 on the ground). Investigators say cracks and metal fatigue caused a left-wing engine-mount failure; the crew had been reassigned to this MD-11F after their original aircraft was grounded for a fuel leak, and the flight carried up to 20,000 packages and about 38,000 gallons of fuel.
