Megaloblastic Anemia Masquerading as Hemolysis in Chronic PPI Therapy: Two Case Reports

Two middle-aged women on long-term proton pump inhibitors presented with severe macrocytic anemia and biochemical signs suggestive of hemolysis (very high LDH, undetectable haptoglobin) but showed a low reticulocyte count. Workup revealed combined vitamin B12 and folate deficiency causing ineffective erythropoiesis that mimicked hemolysis. Vitamin replacement and cessation of PPI therapy led to rapid clinical and hematologic improvement, highlighting that B12/folate deficiency can resemble hemolytic processes and that PPI use may contribute to B12 deficiency.

The Surprising Link Between Vitamin Deficiency and Joint Hypermobility.

Tulane University researchers have discovered a possible genetic cause for hypermobility and related disorders, such as Ehlers-Danlos syndrome, linking it to a deficiency of folate caused by a variation of the MTHFR gene. The discovery could help doctors diagnose hypermobility and hypermobile EDS more accurately by looking for elevated folate levels in blood tests and the MTHFR genetic variant. An existing FDA-approved treatment, methylated folate, has shown promise in improving patients’ symptoms.

The Link Between Vitamin Deficiency and Hypermobile Ehlers-Danlos Syndrome.

Researchers at Tulane University School of Medicine have linked hypermobility and hypermobile Ehlers-Danlos Syndrome to a deficiency of folate caused by a variation of the MTHFR gene. The discovery could help doctors more accurately diagnose hypermobility and hypermobile EDS by looking for elevated folate levels in blood tests as well as the MTHFR genetic variant. The good news is that a treatment already exists, methylated folate, which is FDA-approved and widely available.
