Second-Line BP Drug May Harm Kidneys in Diabetics, Real-World Study Finds
A real-world analysis of 31,031 adults with type 2 diabetes on RAS and SGLT2 inhibitors found that those also taking dihydropyridine calcium-channel blockers had a 33% higher risk of major kidney events (≥40% decline in eGFR or progression to dialysis/kidney transplant) over a median follow-up of 3.5 years. The observational study suggests potential kidney risk with this drug class even in patients on modern kidney-protective therapies, possibly due to how DCCBs affect intraglomerular pressure. Causation can’t be established; randomized trials are needed to guide BP management in diabetic kidney disease.
